A mouth gag (or mouth ball gag) is a device worn during sexual bondage and BDSM roleplay. Gags are usually associated with role play involving bondage, but that is not necessarily the case. The person who wears the gag is regarded as the submissive partner, while the other is regarded as the dominant one. 
People may wear gags for a variety of reasons. Some people derive erotic pleasure from a gag, either in a submissive or dominant role. When combined with other physical restraints, the wearing of a gag can increase the wearer's sense of helplessness and anxiety level within a BDSM scene by rendering them unable to speak during sexual activity, which some people enjoy. Others use gags for punishment and control.

The ball is strapped into the mouth behind the teeth, with the straps going around the head to secure it in place. If the ball is very large, it may be difficult to insert behind the teeth or to remove it. The wearer can still be heard, but it renders their speech completely unintelligible, distorts their facial features, and causes drooling if the wearer attempts to talk.

 

What is BDSM/Bondage? 
BDSM is an overlapping abbreviation of Bondage and Discipline (BD), Dominance and Submission (DS), Sadism and Masochism (SM).

Bondage and Discipline (BD): Bondage and discipline refers to the consensual physical restraint of a sexual partner for the purpose of inflicting pain, punishment, or humiliation. Bondage and discipline is part of Bondage and Discipline, Domination and Submission, Sadomasochism (BDSM), a larger category of sexual activities based in erotic power exchange. Bondage is a common fantasy for both men and women, and it appears that many couples have at one time or another experimented with bondage. Bondage and discipline, however, is practiced by a much narrower portion of the general population and is commonly engaged in by men and women participating in sadomasochism and domination and submission activities.

For the person being bound (generally referred to as the submissive or bottom) bondage can generate sexual excitement from the vulnerability associated with being subject to the will of another and from the mental freedom of having given up control of the situation. Some people enjoy the sensation of struggling against bonds; others may enjoy and derive sexual pleasure from the pain and humiliation inflicted by their partner. Bondage may also incorporate an element of torturous sexual teasing that many find stimulating.

Dominance and Submission (DS): A set of behaviors, customs, and rituals involving the submission of one person to another in an erotic episode or lifestyle. It is a subset of BDSM. 
Physical contact is not necessary, and D/s can be conducted anonymously over the telephone, email, or other messaging systems. In other cases, it can be intensely physical, sometimes crossing into sadomasochism. In D/s, both parties take pleasure or erotic enjoyment from either dominating or being dominated. Those who take the superior position are called "dominants"—Doms (male) or Dommes (female)—while those who take the subordinate position are called "submissive"—or subs (male or female). A switch is an individual who plays either role. Two switches together may negotiate and exchange roles several times in a session. A dominatrix is usually a female sex worker who dominates others for pay.

Sadism and Masochism (SM): The giving or receiving of pleasure from acts involving the receipt or infliction of pain or humiliation. Practitioners of sadomasochism may seek sexual gratification from their acts. While the terms sadist and masochist refer respectively to one who enjoys giving and receiving pain, practitioners of sadomasochism may switch between activity and passivity.
